Home visit membership also includes all home health or hospice care coordination and paperwork, also includes any phone, zoom or in-clinic visits necessary for care transitions
* Other clinics add additional charges for rapid lab testing, lab draws, procedures, or specialized care. We do not add additional charges for weight loss management, ADHD care, menopause care or pain management.
- We maintain a limited number of memberships in order to ensure the highest standard of care.
- There is a non-refundable enrollment feeof $350 per patient (one-time fee, due at signup). Monthly membership fee starts on the day of your initial visit (scheduled after you signup). - Please give 30 days written notice for cancellation of membership. The value in the DPC relationship grows with time. The longer you are a member in our practice, the more value you get. If you decide to cancel, you just let us know. If you cancel and then decide to rejoin, you will be asked to pay the enrollment fee again.
